Our Frequently Asked Questions (FAQ) page also offers answers to common questions about OneNet PPO and about providing services to OneNet Participants.
Help Make Sure Our Participants Can Find You
OneNet Participants rely on our online directory to find OneNet participating providers like you. We appreciate your assistance in making sure the information about you that appears in our online directories is current and correct.
If you haven't done so recently, please take a moment to search for your name to make sure the contact information we are sharing with our OneNet Participants is correct. If a change or update is needed, you can complete the Physician/Provider Demographic Update form and fax it back to us at the number listed on the form.
This form will help you:
- Share corrections or updates to your current office addresses, phone numbers, specialties or other directory information.
- Inform us of providers added to or leaving a group.
- Tell us if you are not accepting any new patients. (If you are closing your practice to new patients, please remember to notify us again when you are once again accepting new patients).
- Need to have a listing removed from the directory because you are a hospital-based physician, are part of a hospital-based group, or practice in a specialty that patients do not usually contact directly.
If you believe your name needs to be removed because you no longer participate in our network, or if you are unable to find your name in our directory, please contact your UnitedHealthcare Network Account Manager or OneNet Professional Services at 1-800-342-3289.
