Discharge Planning and Case Management
By using discharge planning and/or case management, OneNet participants receive assistance in arranging the appropriate
services to meet their recovery needs. Our case management specialists can help customers arrange for:
- Home care (e.g., skilled nursing visits, physical therapy,
occupational therapy, speech therapy and home infusion)
- Durable medical equipment
- Skilled nursing facility placement
- Medical rehabilitation placement
- Hospice care
For assistance, customers can call our Case Management Department Monday through Friday, 8:00
a.m. to 5:30 p.m. at 1-800-962-0643 and ask to speak to a case management
